'Lessons must be learned from Manchester Arena bombing failings'

The ultimate responsibility for the Manchester Arena bombing lies with Salman Abedi.

While we should never forget that, it is heartrending for the families who lost loved ones to learn that there were several missed opportunities to prevent the attack.

These included the failure of two stewards to respond effectively to concerns raised by a member of the public, the lack of adequate policing, CCTV black spots and poor security.

It is essential that lessons are learned to try to ensure such an event never happens again.

That is the least the families of the 22 people who died deserve.
